Katy Medical Complex Pharmacy – Text Message Opt-In

We send text messages only to patients who opt in. We do not use paper forms. Patients opt in in one of two ways:
1. Digital check-in form on an Android tablet at the pharmacy counter
The patient enters their name, date of birth and mobile number, and may check this optional box, which is unchecked by default:

Links to our Privacy Policy and Terms & Conditions appear directly below the box. Patients who leave it unchecked receive no texts. Screenshots of the tablet form and the screen shown after submitting are below.
All text messages are sent by one business: Leviant LLC, doing business as Katy Medical Complex Pharmacy. We collect consent only for this business. We never collect consent for affiliates, partners or other companies, and we never share or sell phone numbers or opt-in data.
2. Verbal consent (in person or by phone)
Staff read this script:
"Would you like texts from Katy Medical Complex Pharmacy at this number, including verification codes, order-ready notifications, ID upload links and customer care follow-ups? Msg frequency varies. Msg & data rates may apply. Reply STOP to opt out, HELP for help. This is optional."
Only if the patient says yes, staff record the phone number, date, time and staff member's name. Verbal opt-ins do not receive review requests.
Confirmation text
After opting in by either method, the patient receives: "Katy Medical Complex Pharmacy: You're subscribed to patient texts. Msg frequency varies. Msg & data rates may apply. Reply HELP for help, STOP to opt out."
