These Terms & Conditions govern your use of katymedicalpharmacy.com, our pharmacy services, and our text messaging program, all provided by Leviant LLC, doing business as Katy Medical Complex Pharmacy ("Katy Medical Complex Pharmacy," "we," "us," or "our"). By using our services or opting in to our text messages, you agree to these terms.

1. Our services

Katy Medical Complex Pharmacy provides pharmacy services to its patients. Information on this website and in our messages is general information only and is not medical advice. Always follow the instructions of your doctor or pharmacist. Do not use text messages for emergencies. Call 911.

2. SMS Terms

Katy Medical Complex Pharmacy Patient Text Messages.

Program description

Katy Medical Complex Pharmacy sends text messages to patients who have opted in. Messages may include:

• One-time verification codes for signing in to your account
• Notifications that an order is ready for pickup
• Secure links to upload identification documents
• Customer care follow-ups
• Occasional requests to leave a review after your visit

Our messages never include medication names or details about your health. All text messages are sent by one business: Leviant LLC, doing business as Katy Medical Complex Pharmacy. We collect text message consent only for this business, never for affiliates, partners, or other companies.

How you opt in

• Online sign-up form: enter your name and mobile number on our sign-up form at katymedicalpharmacy.com/opt-in and check the consent box. The box is never pre-checked. The same form is used on the tablet at our pharmacy counter.
• Verbal consent: at the counter or by phone, a staff member reads you our text message consent script, and you agree out loud. We record your consent with the date, time and staff member's name. Verbal opt-ins receive account and service texts only, not review requests.

Consent to receive text messages is not a condition of any purchase or of receiving pharmacy services.

Message frequency varies based on your activity with the pharmacy.

How to opt out

You can cancel the SMS service at any time. Text STOP to the number that messaged you. After you send STOP, we will send you one message confirming that you have been unsubscribed, and you will no longer receive messages from us. To join again, reply START or sign up again at katymedicalpharmacy.com/opt-in.

Carrier liability

Carriers are not liable for delayed or undelivered messages.

Verification codes

Never share a verification code with anyone. Katy Medical Complex Pharmacy staff will never ask you for a code sent to your phone.

For privacy-related questions, please see our Privacy Policy. We do not sell or share your SMS opt-in data or personal information with third parties for marketing purposes.

3. Reviews

Leaving a review is entirely voluntary. We do not offer payment, discounts, or other incentives in exchange for reviews, and your pharmacy services are not affected by whether you leave one.

4. Acceptable use

You agree not to misuse our website or services, including by trying to access another person's account, uploading documents that are not your own, or interfering with our systems.

5. Limitation of liability

To the fullest extent permitted by law, Leviant LLC and Katy Medical Complex Pharmacy are not liable for indirect, incidental, or consequential damages arising from your use of our website or text messaging program, including delayed or undelivered messages.

6. Governing law

These terms are governed by the laws of the State of Texas, without regard to its conflict of law rules.

7. Changes to these terms

We may update these terms from time to time. The updated version will be posted on this page with a new effective date.
